The National POLST (Physician Orders for Life Sustaining Treatment) Paradigm is hosting a webinar series to present a new POLST toolkit they have developed to honor patient wishes across the care continuum. The toolkit is an online document which, for a given care setting, describes the POLST Paradigm and best practices for implementing it within a practice. Resources such as sample policies and other reference materials are included. The free webinar series, which starts tomorrow, April 6, and continues into June, includes six webinars: an overview and 5 others corresponding to various care settings in which POLST is important (i.e. Primary Care, EMS, In-patient Hospital, Nursing Homes and Hospice).
