New Death Certificate Forms Launch in 2014

Effective January 1, 2014, physicians will need to utilize a new death certificate form for any patients who pass away from natural causes; the old form should be used for any deaths that occur in the remainder of the 2013 year. The DHHS Vital Records department has created a training section online that spells out the changes to the death certificate forms. Access the Vital Records training by using the login information below.

Username:  vrdeath
Password:  death

Once you have logged in, click on the “Medical Certifiers” tab to see the specific fields that have been changed or added. To view the new form in its entirety, visit the “Revised NC Death Certificate” link on the right of the page.
